Three Reasons Why Spinal Arthritis Is More Difficult To Treat Than Hip or Knee Arthritis

The joints of the spine are called facet joints. They are located on both sides of the spine at every level up and down the spinal column all the way from the skull down to the sacrum.

Each of these joints combines to allow humans an incredible range of motion of the spines, permitting bending and twisting to an exceptional degree. Unfortunately though, each of these joints has cartilage and it has the potential for arthritis either due to age with loss of cartilage or trauma with damage to cartilage and consequent arthritis pain.

Spinal arthritis is more difficult to treat than that of an extremity such as the hip or the knee. Here are the three main reasons why this is the case.

1. The number of joints in the spine is much higher. When you're dealing with hip or knee arthritis, there is one on each side. When you're dealing with spinal joints, there is one on each side at every level. Each of them is prone to developing arthritis and pain. even though the joints are significantly smaller, arthritis at any one facet joint can cause just as much pain as arthritis in a knee or a hip joint.

Typically when a person develops arthritis in a facet joint, he or she has arthritis in multiple facet joints which would need to be treated. Figuring out which is causing the pain and treating appropriately is much more difficult in a spinal arthritis situation than for the hip or knee.

2. Figuring out the source of the pain is more difficult. Even the best trained spine doctors who are board-certified can only tell people 50% of the time exactly why their back hurts. This is one of the shortcomings of back pain treatment, that the world of modern medicine is simply farther along when it comes to hip or knee arthritis than for that of the spine.

What this means is that if a pain management doctor does an injection into one of the facet joints of the lumbar spine, it may hide part of a person's back pain, but a decent amount of the time they will still have significant residual pain. Pain management doctors have developed some more specific techniques to delineate the source of a person's pain, including facet joint injections and medial branch blocks as diagnostic tools. This will continue to improve, however, currently it makes spinal arthritis more difficult to deal with than that of the hip or knee.

3. Surgical outcomes for spinal arthritis treatments are not as good as that of total joint replacement for the hip or knee. Out of all the surgeries done for quality-of-life in the world, total hip and total knee replacement are in the top five overall. They are unbelievable game changes when it comes to decreasing pain and increasing function.

The same can not be said for surgery for spinal arthritis. Although artificial disc replacement has been in existence since 2004, it has not been shown to be as good of a procedure as extremity joint replacement. In addition, there is no FDA approved procedure for replacing the facet joints in the back of the spine. What this means is that if a person undergoes surgery for facet arthritis, it will involve a spinal fusion. Satisfactory results after these treatments are in the 50 to 75% range, whereas knee replacement surgeries are typically over 90% satisfactory with vacancies.

The hope is that with modern medical techniques advocating, diagnosing the exact areas of spinal arthritis will become more exact and the results will be better with surgery. For now, nonsurgical pain management treatment is actually very good at reducing pain. it just takes more diagnostic tools and thought-provoking maneuvers to make sure the proper levels are being treated with the appropriate procedures.

Investigative Tests

X-rays are among the most common investigative tests performed to assess the severity of the elbow fraction. X-rays are almost always required so that the farming physician can determine what kind of fracture they are dealing with. Assessment must be made to see if the bones have moved out of place or remain intact.

For children, a careful assessment must be made because the bones are still growing and developing. A doctor may require x-rays being done of both arms in order to compare bone formation.

Non-Surgical and Surgical Treatment Options

When there is little to no bone displacement, surgery may not be required. This means that the elbow fraction was clean and that the bones did not shift around. Non-surgical treatment consists of immobilization of the arm by a cast or splint for several weeks. Through this time, the doctor may order x-rays at various stages to ensure healing is taking place and bones are staying aligned.

In severe elbow fractures, where the bones are crushed or have moved around, surgery may be required. The bones must be manipulated back into place via surgical procedures. These procedures may include the use of pins, screws and wires designed to position the bones and hold them in place while the elbow heals. A cast is worn for several weeks following the procedure.

5 Steps to Relieving the Pain in Your Jaw

Jaw Pain, or TMJD (which stands for tempomandibular joint disorder), as a common ailment that affects millions of Americans each year. Most people who get jaw pain, get it for a short period of time due to an intense amount of stress in their lives or injury. Some people get more chronic jaw pain, which can be particularly difficult to treat and manage as there are not many proven methods of treatment for chronic TMJD that have been effective for more than just a small number of people.

Step 1: Ice. Like most injuries or swelling, ice can numb the pain and help facilitate healing. A twenty minutes on, twenty minutes off cycle should be sufficient. The more you can do this, the faster the healing will be.

Most jaw pain is caused by swelling of the jaw joint, due to excessive clenching / grinding (bruxism) or tension. Step 4: If your jaw pain has not been healed by now, you must start moving into long-term treatment and management. One simple way to start this long-term management is to start jaw exercises. Simple jaw exercises, like moving your jaw side-to-side with your hand pressed against it, can help strengthen your jaw and give you more control over it.

Step 5: Get to the root cause of the pain. If jaw pain is becoming an issue over many weeks or months, there is usually an underlying cause. Often times the undering cause is stress or clinging (often times at night while you do not even know you're doing it). The Web version of the Encyclopedia Britannica defines pain as – “A complex experience consisting of a physiological (bodily) response to a noxious stimulus followed by an affective (emotional) response to that event. by influencing it to withdrawal from harmful stimuli. It is primarily associated with injury or the threat of injury, to bodily tissues. ”

Pain is an individual sensation that can be described or defined by the person having it. It may cause distress and discomfort, it is usually described as: aching, pinching, throbbing or stabbing. We may distinguish between two basic types of pain – acute and chronic.

Acute pain lasts a relatively short time. It is a signal that body tissue is being injured. The pain generally disappears when the injury heals. Acute pain results from disease, infection or tissue injury. It may appear suddenly, such as after trauma trauma and may be accompanied by emotional or anxiety distress. The cause of acute pain can usually be diagnosed and treated accordingly. In certain cases it may become chronic.

Chronic pain may range from mild to severe and lasts usually for long periods of time, more than three months. It is associated with the disease itself. Chronic pain may get worse by psychological or environmental factors.

The cause of chronic pain is not always evident. In certain cases, it may be associated with chronic conditions such as: arthritis, fibromyalgia or lupus with symptoms such as: swollen joints, unexplained fever, extreme fatigue, sleep problems or red skin rash. Chronic pain syndromes, in particular, are complex and their effective treatment often involved coordinated, multidisciplinary consultation.

In contrast to acute pain, chronic pain can be mysterious, intractable and is often very expensive to treat. The complexity of chronic pain stems from the fact that it is a bio-psycho-social condition, which occurs in various forms.

Since pain is a bio-psycho-social condition, all aspects of the condition must be treated. Assuming that a condition is “all in the patient's head” makes the mistake of overlooking possible real pain.

On the other hand, failure to assess the psycho-social factor can also lead to longer recovery. The complex nature of chronic pain disorders makes it impossible for a single professional to treat it successfully.

We may distinguish between peripheral and central pain.

Peripheral pain originates in the peripheral nerves or in muscles, usually via trauma.

Central pain arises from Central Nervous System (CNS) pathology or dysfunction. This is primarily due to structural changes in the CNS, such as: spinal cord injury, multiple sclerosis, stroke and epilepsy.

Inhibition of pain is important and needed especially when our safety is more important, like when we are running away from dangerous situation. The purpose of pain is to tell us via our brain when something needs to be done about a damaged area. The brain will assist us wherever to pay attention to the painful area or honor it.

This information is transmitted by the brain and travels to the spinal cord or brain stem via electrical impulses in fibers of spinal or certain cranial nerves. Those signals pass electrically to higher CNS levels.

Therefore, monitoring those signals in real time may be used as essential parameters in our effort for detecting and verifying pain.

Pain and Gender. Recent studies using Positron Emission Tomography (PET) brain scans of patients during pain stimuli shown different brain responses between men and women.

Several areas of male and female brains responded differently to the same pain stimuli. Female's brain shown more activity in emotion related centers where males responded in the cognitive or analytical regions. Those differences may refer to our evolution process and the different social tasks of males and females.

Women often have high pain experience levels but lower pain tolerance. Their sensitivity to pain is affected by many factors such as biological, inherited conditions and hormone levels.

Pain and Animals. The presence of pain is by the observation of change from normal behavior.

Pain may be evident as a limp or a change in gait, withdrawal or protection of an injured part, abnormal postures, licking, rubbing or scratching at an area. Signs of pain and distress particularly to rodents include eating too much, chewing toes and feet.

Signs of pain may be subtle such as a change in respiration, referral to move, apprehension, sudden aggression, inability to rest or sleep normally, or a worried or anxious expression.

Types Of Fractures A Pediatric Orthopedic Surgeon Treats

Fractures are among the many musculoskeletal conditions that a pediatric orthopedic surgeon beats. Many people have become so identified with their pain that they are defined by it. Others have found emotional resolutions from traumas like sexual abuse, mental abuse, neglect and more yet they do not quite feel done. At the thinking level they may have it all worked out but at the cellular level there are movement patterns from the trauma that must be addressed.

Stroke is one of the most destructive traumas that more and more people experience today. That is how we eliminate pain emotionally, intellectually and physically.

It's very common for people to suffer from sore and aching muscles especially after doing activities where muscles normally do not get that much of a workout on a daily basis. 7 Ways to Protect Your Spine From Injury

Our spine consists of three parts namely the cervical spine, thoracic spine and the lumbar spine. These different parts bear the grunt of our weight and are totally defenseless to injuries and strains. However, there are ways to protect our spine by taking note of the following.

Stretching

Doing regular back stretches correctly is crucial to the protection of the spine. Stretching relaxes the back and helps the body to recover properly and tone up. Stretching can help ease tension and keep the spine in the most comfortable position.

Exercise

Exercising helps to strengthen the back muscles which in turn will help to provide an even better support to the spine. Always try to stand upright and sit upright in the right posture as poor future will put a huge amount of strain on the spine.

Ergonomics

Always make sure that ergonomic sitting is done in your office. Make sure that the backrest of your chair is tilted in a slightly reclined position. Both of your feet must be firmly on the ground. Always kneel on one of your knees and bring the item to your waist level. Use the abdominal muscles to support your back and breathe out and lift up the item at the same time.

Our spine is very fragile and is easily injured. Always make sure that you are not doing activities that will put tremendous stress and pressure on it and always protect it.

Most love addicts and many other varied types of addict are survivors of childhood trauma. Childhood trauma in and of itself is often misunderstood and misrepresented. This is usually because the word trauma is associated with violence.

The fact is that this is not necessarily true. Trauma can come in many forms and is determined by either the intensity of the threat or the weakness and vulnerability of the defences.

It is well recognized that children are particularly vulnerable, not unnecessarily because they are weak but because of their inherent limited understanding.

This lack of capacity to understand often means that they are vulnerable to situations that an adult would easily deal with.

Often the wounds we suffer in childhood are often still with us in adulthood and leads to a condition of learned helplessness. Learned helplessness creates dysfunction and leads to emotional confusion and stunted creativity.

It is characterized by self sabotage and is frustrating and can lead to depression and despair.

The normal journey of a human being is that we grow from childhood to full adulthood and as we grow mature physically, mentally and emotionally. This maturing process enables us to live with creative power, but if we do not mature emotionally then we are living in the emotional reactions of a frightened child.

This is the condition that constituents learned helplessness and recovery from it requires that you heal the residual wounds of the original trauma.

Much therapy in this area attempts to overcome the condition by trying to understand what caused the original trauma. There is much evidence to show that this approach is often less than successful. The problem is that the ego defences of the frightened child are so powerful that letting go of them is very frightening. What must be remembered is that these defenses are there for one very important reason; they have worked, at least to the satisfaction of the child.

Human beings are programmed to repeat what works best. This is a part of our evolutionary journey. Re-programming the ego takes time and lots of trust. Trusting in a new way quickly is very challenging to many people whose trust has been undermined during the original trauma.

All 12 Step recovery programs are of the 'educational variety'. What needs to be educated is the damaged ego. All healing is really a release from the past and this enables us to live with freedom in the present without the excess baggage of past hurts weighing us down.

Trauma is any experience that interferes with a child's sense of security and if this sense of insecurity is transported into adulthood, it leads to much emotional suffering. Many people living in this condition believe that their suffering is coming from their environment, but it is in fact coming from within themselves because of their outdated obstructive response to their environment.

An honest inventory of their instinctive responses will show the reality of their predicament and gives them the much needed power to change. This is the basis of spiritual or emotional recovery.

In short, the adult with the childhood responses needs to learn how to respond in a more adult like fashion. This will immediately lessen the psychic pain and allow recovery to take place.

The problem with the childhood responses is that they once worked very well according to the limited understanding of the frightened child, but now that you have grown to adulthood these responses have become a liability and rather than solve the problem of the trauma they now prolong it . They are both the cause of the problem and the consequence of it.

This vicious circle must be broken in order for real change and progress to be made. Trusting in this process and taking action will lead to a fundamental personality change, which will of itself, lead to a more satisfying and fulfilled life.
